The Community Assessment Program (CAP) strives to keep youth at home and out of juvenile justice and human services systems by connecting families with supportive services to promote safe, healthy and happy kids. Young people receive early intervention screening and assessment to identify factors contributing to concerning behavior and factors mitigating risks. Referrals are accepted from parents and professionals.

Signs of concerning behavior include:
- Substance use
- Truancy
- Delinquency
- Concerning behaviors in the classroom and at school
- Concerning behaviors at home
- Youth running away from home
- Traumatic experiences
- Mental health concerns
Who can refer a youth to the Community Assessment Program?
Anybody can make a referral through our Community Assessment Program. People who tend to make referrals are:
- Parents or guardians
- School counselors
- Teachers
- Child welfare case workers
- Officers
What happens if I refer a youth to the Community Assessment Program?
- We offer free phone consultations if you think a youth may be in need of or benefit from an assessment
- This is a voluntary service for the youth and his or her family.
- After the phone consultation with the adult, a member of the JAC will conduct an assessment with the youth in person at the Family Resource Pavilion or a place in the community, like a school or library
- There are no fees for referrals or assessment services
The CAP assessment includes:
- Screening for immediate medical needs
- Behavior health screening (including mental health and substance use)
- Assessment of risks, strength and needs
- Case-planning and short-term follow-up
